Student immunization information
All students entering the School of Optometry must meet the immunization requirements of the university and Indiana Code 20-12-71. IUSO students will not be allowed to enroll in classes until immunization requirements have been fulfilled.
Read the Student Immunization Policy.Indiana state law and Indiana University policy require students to document immunizations in English. Please download, complete, and return the School of Optometry Student Immunization Form to the Student Administration Office, OPT room 210, prior to the start of your first semester. For those students who have a medical condition that precludes receiving vaccines, please download the Medical Contraindication Statement. Make sure to keep your TB test updated. You are required to have the test repeated yearly. If you have not received one in the past 12 months, a hold will be placed on your record preventing class registration. The Health Center does come through in the early spring to offer the test but the timing is not correct for everyone. For example, if you had a TB test done in Summer 2016, you will need to get another one completed in Summer or Fall of 2017 and cannot wait until Spring 2018 to get the test done with the Health Center. You are required to keep your tetanus booster updated as well – make sure you’ve had one within the past ten years. You will be contacted and a hold will be placed on your record if your tetanus documentation is outdated.
